- Build and maintain working relationships with community health promoters, local leaders, schools, faith-based organisations, and other stakeholder groups to expand the hospital’s reach.
- Develop and manage referral pathways between community-based health actors and the hospital, ensuring smooth patient handoffs and follow-up communication.
- Act as a liaison between the community and the Director of Clinical Services, relaying insights on local health challenges and barriers to care.
- Maintain an up-to-date database of community contacts, outreach activities, and referral outcomes to track engagement and measure impact.
- Support the creation of health education materials and community-facing messaging that aligns with the hospital’s clinical priorities and brand.
- Represent the hospital at community meetings, health committee sessions, and partner forums to strengthen its presence and credibility.
- Identify gaps in access to services and propose practical solutions to improve community utilisation of hospital care.

- Current registration with the relevant professional health regulatory body.
- At least three years of hands-on experience in a hospital, clinic, or community health programme environment.
- Demonstrated ability to build trust with diverse groups, communicate clearly, and organise multiple activities simultaneously.
