Role Overview
This community health role connects local households with preventive care, health education, and services delivered through nearby health facilities. Through outreach, home visits, and follow-up, you will help residents make informed health choices while supporting accurate reporting and coordinated care.
Key Responsibilities
- Encourage community participation in health campaigns, immunization activities, and other outreach services.
- Visit households to discuss practical steps for better hygiene, sanitation, nutrition, and disease prevention.
- Support programs focused on maternal, newborn, child, and adolescent health.
- Lead or assist with community education on priority health topics.
- Encourage adherence to treatment plans and help connect residents with referrals and follow-up care.
- Gather and document community health information using approved Ministry of Health tools, and submit required reports.
- Keep clear records of outreach work, community activities, and referrals.
- Coordinate with dispensary staff and community health volunteers, follow applicable Ministry of Health guidance, and protect confidentiality during all activities.
Requirements & Qualifications
- Certificate or diploma in Community Health or Public Health from a recognized institution.
- Registration with the relevant professional body, where applicable.
- At least one year of experience in community health or public health programs is preferred.
- Ability to communicate health information clearly, work respectfully with community members, and maintain accurate records.
- Willingness to take part in training, meetings, and ongoing professional development.
