Description:
Chronic Care Staffing is currently hiring a highly organized and detail-oriented Scheduling Coordinator to support its patient referral and care coordination operations. This is a fully remote position based in the United States, offering the flexibility to work from anywhere while contributing to essential healthcare processes.
The role focuses on ensuring accurate handling of patient information, referral documentation, and coordination between healthcare providers and external specialists. The ideal candidate will thrive in a structured yet fast-paced healthcare administrative environment with a strong emphasis on accuracy and compliance.
Key Responsibilities:
• Review electronic health records (EHRs) to gather and verify patient information
• Retrieve and organize essential documentation such as demographic sheets, insurance cards, and office visit notes
• Prepare and submit referral packets to external medical specialists
• Process referrals through fax, phone, and secure healthcare portals
• Identify appropriate referral destinations using internal specialty resource guides
• Ensure referral packages are complete, accurate, and compliant with standards
• Collaborate with follow-up and care coordination teams
• Cross-train on additional workflows to support operations
• Monitor documentation quality and correct missing or incorrect information
Requirements:
• 1–2 years of experience in medical office administration or healthcare support roles
• Comfortable working with Electronic Health Records (EHR) systems
• Strong computer and data entry skills
• Excellent attention to detail and accuracy
• Ability to manage multiple tasks and prioritize workload
• Strong written and verbal communication skills
• Ability to work independently in a remote environment
