Role Overview
This position places a community health nurse at the heart of primary care delivery, working directly with patients, families, and the wider community to improve health outcomes. You will manage the full cycle of care—from initial assessment through treatment, education, and follow-up—while also supporting facility-level activities like immunizations, maternal health services, and disease surveillance. Your work is essential to ensuring that vulnerable populations receive timely, compassionate, and professionally sound nursing care in line with national health protocols.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans, implementing nursing interventions while continuously evaluating and adjusting them based on patient responses.
- Deliver a wide range of clinical services, including Integrated Management of Childhood Illnesses (IMCI), routine immunizations, prevention of mother-to-child transmission (PMTCT), antenatal care, and safe delivery support.
- Provide health education and counseling to patients, families, and community groups, addressing identified health needs and promoting preventive practices.
- Identify cases requiring specialized or advanced care and make timely, appropriate referrals to ensure continuity of treatment.
- Manage patient admissions, coordinate admission documentation, and initiate discharge planning in collaboration with the wider care team.
- Maintain accurate, up-to-date records of each patient's health condition, treatment provided, and care outcomes, ensuring confidentiality and completeness.
- Keep the clinical environment clean, organized, and safe, adhering to infection control and safety standards at all times.
- Collect, compile, and report routine health data to support facility planning, quality improvement, and community health monitoring.
Requirements & Qualifications
- Hold a Diploma in Kenya Registered Community Health Nursing from a recognized institution.
- Be registered as a Community Health Nurse with the Nursing Council of Kenya.
- Possess a valid practicing license from the Nursing Council of Kenya, with no lapse in licensure.
