Role Overview
You will be an integral part of the hospital pharmacy team, ensuring that every patient receives the correct medication, the right dose, and the necessary guidance to use it safely. On a typical day, you will review prescriptions, prepare and dispense medicines, collaborate with doctors and nurses on treatment plans, and maintain accurate pharmaceutical records. Your attention to detail directly impacts patient outcomes and helps the facility uphold its standards of safe, effective medication management.
Key Responsibilities
- Review and interpret prescriptions to confirm dosage, drug interactions, and contraindications before dispensing.
- Prepare and dispense medications accurately while maintaining proper labeling, packaging, and documentation.
- Provide clinical pharmacy services on hospital wards, including medication reconciliation and therapeutic monitoring.
- Advise patients and caregivers on proper medication use, storage, side effects, and adherence strategies.
- Maintain and update patient medication records using the facility’s Health Management Information System (HMIS).
- Monitor medication stock levels and help manage inventory to prevent shortages or expired products.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize treatment plans.
- Report adverse drug reactions and participate in pharmacovigilance activities in line with regulatory requirements.
Requirements & Qualifications
- Bachelor of Pharmacy or Diploma in Pharmaceutical Technology from a recognized institution.
- At least three years of post-internship work experience, ideally within a hospital pharmacy setting.
- Full registration and a valid practicing license from the Pharmacy and Poisons Board.
- Proficiency in computer use, especially with Health Management Information Systems and electronic medical records.
- Strong verbal and written communication skills, with the ability to interact effectively with patients and healthcare staff.
- Good interpersonal skills and a collaborative approach to working within multidisciplinary teams.
