Role Overview

This role coordinates the entire patient journey into a home health programme—from initial identification and home-readiness checks through to discharge planning and family teaching. Day-to-day, you will supervise field nurses, run clinical audits, and ensure staffing levels keep care seamless. Key Responsibilities

• Oversee patient enrolment into home health services: assess home environments for safety and suitability, develop individualised care plans, and educate patients and families on managing conditions at home.
• Provide clinical leadership to the nursing team by conducting supervisory home visits, reviewing documentation, and ensuring evidence-based, patient-centred care is consistently delivered.
• Support staff development through competency monitoring, one-on-one coaching, and coordination of ongoing training and professional growth initiatives.
• Act as the central liaison among patients, families, caregivers, and multiple healthcare providers to ensure smooth, coordinated care across the entire continuum.
• Manage daily staffing and service coverage to guarantee uninterrupted care, while maintaining up-to-date licensure, certifications, and workforce compliance records.
• Build and sustain referral relationships with hospitals, physicians, and community stakeholders to meet patient enrolment targets and expand programme reach.
• Lead quality improvement projects, patient safety audits, and risk management activities, ensuring full compliance with organisational policies and regulatory standards.
• Track clinical outcomes, team performance, and operational metrics; produce regular reports that inform service improvements and measure programme effectiveness.

Requirements & Qualifications

• Registered Nurse qualification from a recognised school of nursing or university.
• Valid, current licence to practise from the Nursing Council of Kenya.
• Certification in Basic Life Support (BLS).
• Minimum five years of clinical nursing experience in an acute care hospital setting.
• Previous experience in home health services is considered a strong advantage.
• Demonstrated leadership ability and proven care coordination skills.
• Strong critical-thinking and analytical capabilities.
• Comfort working within multidisciplinary teams and contributing to shared decision-making.
• Excellent organisational and time-management skills, with the ability to prioritise competing demands in a fast-paced environment.
• Self-driven and capable of working independently while maintaining accountability for outcomes.
