Curana Health is seeking a dedicated Care Navigator to support healthcare providers and care managers with non-clinical patient support activities. This remote position is ideal for professionals with experience in medical administration, electronic health records, and patient communication.
The successful candidate will assist patients, caregivers, and healthcare teams by managing records, coordinating appointments, supporting provider operations, and ensuring smooth communication throughout the care process. The role requires strong organizational skills, attention to detail, and the ability to work effectively in a virtual healthcare environment.
Key Responsibilities:
• Respond to patient and caregiver inquiries through telephone and patient portal communication.
• Assist patients with scheduling follow-up appointments with healthcare providers and specialists.
• Manage electronic health records (EHR) and maintain accurate patient information.
• Coordinate documents required for provider review and approval.
• Support provider orders and escalate findings when necessary.
• Maintain patient rosters for Advanced Primary Care Management (APCM) and GUIDE programs.
• Assist with provider scheduling and prior authorization processes.
• Obtain patient records and diagnostic test results as required.
• Answer and manage incoming calls professionally.
• Collaborate with interdisciplinary care teams to ensure effective communication.
Requirements:
• High school diploma or equivalent.
• Minimum 1 year of experience working in a medical office, senior living community engagement, or related healthcare field.
• Minimum 1 year of experience with Electronic Health Record (EHR) documentation or practice management tools.
• Strong understanding of medical terminology.
• Ability to interpret medical records, lab results, and appointment notes.
• Excellent organizational and time management skills.
• Ability to manage multiple tasks and prioritize responsibilities effectively.
• Proficient computer skills with the ability to adapt to different technology platforms.
• Reliable high-speed internet connection for remote work.
• Strong knowledge of:

Preferred Qualifications:
• Experience with virtual triage.
