Job Description

Role Overview

This position places you at the centre of safe and effective medication use within a hospital pharmacy environment. On a typical day, you will verify prescriptions for clinical appropriateness, prepare and dispense medicines, and counsel patients on how to get the most from their treatment. Your attention to detail and clinical judgement will directly influence patient outcomes and help the hospital maintain high standards of pharmaceutical care.

Key Responsibilities

  • Review prescription orders for accuracy, therapeutic suitability, correct dosage, and potential drug interactions or contraindications before dispensing.
  • Prepare and dispense medications in accordance with legal and professional standards, ensuring correct labelling and patient-specific instructions.
  • Offer medication counselling to patients and caregivers to promote adherence, proper storage, and understanding of possible side effects.
  • Participate in ward rounds and clinical discussions, advising medical and nursing staff on rational drug therapy and dosage adjustments.
  • Monitor and manage pharmaceutical stock levels, including expiry checking, reordering, and maintaining cold-chain integrity where required.
  • Document and report adverse drug reactions and medication errors to support pharmacovigilance and continuous quality improvement.
  • Maintain accurate, up-to-date patient medication records using the hospital's Health Management Information System.
  • Contribute to hospital initiatives on antimicrobial stewardship and medication safety protocols.

Requirements & Qualifications

  • Degree in Pharmacy or Diploma in Pharmaceutical Technology from a recognised institution.
  • A minimum of three years of post-internship working experience, preferably within a hospital or clinical pharmacy setting.
  • Current registration and a valid practising license from the Pharmacy and Poisons Board.
  • Demonstrated competence in computer use, particularly with Health Management Information Systems and pharmacy software.
  • Strong communication and interpersonal skills, enabling clear interaction with patients, caregivers, and multidisciplinary teams.
