Job Description

Role Overview

This position places a licensed pharmacist or pharmaceutical technologist at the heart of hospital-based medication management. On a typical day, you will review and dispense prescriptions, verify dosing and drug interactions, and counsel inpatients or outpatients on safe medication use — all while maintaining accurate records within the hospital’s Health Management Information System (HMIS). Your work directly supports patient safety, treatment effectiveness, and the pharmacy’s compliance with national regulatory standards.

Key Responsibilities

  • Review prescription orders for appropriateness, accuracy, and potential drug interactions before dispensing; clarify ambiguous orders with prescribers when necessary.
  • Prepare and dispense medications accurately, including unit-dose packaging and compounding of basic sterile or non-sterile preparations where applicable.
  • Provide clear medication counseling to patients and caregivers, covering dosage, side effects, storage, and adherence support.
  • Conduct medication reconciliation during admission, transfer, and discharge, collaborating with medical and nursing teams to resolve discrepancies.
  • Monitor patient responses to therapy and escalate any adverse drug reactions or medication-related problems to the appropriate clinical staff.
  • Maintain complete and up-to-date patient medication records, inventory logs, and controlled drug registers using the hospital’s HMIS and paper-based systems as required.
  • Support pharmacy stock management — receiving, storing, and rotating pharmaceuticals; identifying near-expiry or expired items; and minimizing waste.
  • Contribute to ward rounds or clinical meetings when required, offering pharmacotherapy advice and ensuring high-risk patients are closely followed up.

Requirements & Qualifications

  • A Bachelor of Pharmacy degree or Diploma in Pharmaceutical Technology from a recognized institution.
  • A minimum of three years of practical, post-internship pharmacy experience, ideally gained in a hospital or clinical setting.
  • Full registration and a valid practice license from the Pharmacy and Poisons Board (PPB), without restrictions.
  • Solid computer proficiency, especially in using Health Management Information Systems (HMIS) or similar electronic medical/pharmacy record platforms.
  • Strong oral and written communication skills, enabling clear interaction with patients, prescribers, nurses, and other healthcare staff.
  • Good interpersonal and teamwork capabilities, with the ability to work effectively under pressure in a fast-paced clinical environment.
