Description:
Aligned Modern Health is seeking a motivated and patient-focused Care Coordinator to join its growing healthcare team. The Care Coordinator will serve as a key connection between patients, clinics, and healthcare providers by managing appointments, responding to inquiries, and ensuring a smooth patient experience. The successful candidate should be organized, personable, comfortable with technology, and passionate about helping people improve their health and wellness.
Key Responsibilities:
• Provide outstanding customer service and build positive relationships with patients.
• Communicate with patients through phone calls, emails, and live chat to assist with scheduling needs.
• Coordinate and schedule new patient appointments across multiple clinics and telehealth services.
• Respond promptly to patient inquiries and provide accurate information.
• Serve as a scheduling liaison between patients, clinic teams, and healthcare providers.
• Support clinic operations by assisting with scheduling, insurance collection, and billing-related questions.
• Maintain accurate patient information and handle confidential data professionally.
• Collaborate with internal teams to ensure patients receive a high-quality healthcare experience.
• Complete administrative tasks while managing multiple priorities in a fast-paced environment.
Requirements:
• Bachelor’s Degree from an accredited 4-year college or university.
• 1+ year of experience in retail, sales, hospitality, customer relations, or a related field.
• Excellent listening, communication, and interpersonal skills.
• Strong customer service mindset with a professional and friendly approach.
• Ability to work effectively as part of a team in a fast-paced environment.
• Strong written and verbal communication skills with the ability to handle sensitive information.
• Excellent organizational skills and attention to detail.
• Ability to manage multiple responsibilities and complete assigned tasks efficiently.
• Comfortable performing daily work using computers, phone systems, and online tools.
• Proficiency with Microsoft Office products and various software platforms.
• Must have reliable high-speed internet access and a quiet workspace for remote calls.
• Strong knowledge of:
• Patient scheduling, healthcare customer service, and care coordination
• Administrative processes, insurance support, and patient communication
• Microsoft Office, healthcare software systems, and remote communication tools
