Role Overview

This position sits at the centre of day-to-day medication management inside a hospital pharmacy, where accurate dispensing, sound clinical judgement and disciplined record-keeping directly shape patient outcomes. The post-holder works alongside prescribers, nurses and laboratory staff to ensure every patient receives the correct medicine, at the correct dose, by the correct route, at the correct time — and that every transaction is captured properly in the health management information system. It is a hands-on role that rewards attention to detail, composure under pressure, and a genuine commitment to safe and ethical pharmacy practice.

Key Responsibilities

• Dispense prescribed and over-the-counter medicines after verifying dose, route, frequency, duration and potential interactions before release to patients or wards.
• Review medication orders for clinical appropriateness and consult with prescribers on substitutions, dose adjustments or therapeutic alternatives where needed.
• Counsel patients and caregivers on correct administration, storage, expected side effects and adherence, with particular attention to long-term and chronic therapy.
• Maintain accurate stock records covering receipts, issues and returns, and monitor stock levels, expiry dates and cold-chain items to prevent shortages and wastage.
• Capture all dispensing and inventory data in the Health Management Information System and prepare the periodic reports required for audits and management review.
• Take part in ward rounds and clinical meetings, contributing medication-related input to multidisciplinary care plans.
• Identify, document and report adverse drug reactions, medication errors and near-misses through the established reporting channels.
• Support antimicrobial stewardship and rational drug use initiatives, and guide interns, students and support staff working in the dispensary.

Requirements & Qualifications

• A Degree in Pharmacy or a Diploma in Pharmaceutical Technology from a recognised institution.
• A minimum of three years of working experience after completing internship, with hospital-based experience preferred.
• Current registration and a valid practising licence issued by the Pharmacy and Poisons Board.
• Practical computer proficiency, particularly in health management information systems, including accurate data entry and generation of routine reports.
• Working knowledge of good dispensing practice, drug regulation and pharmaceutical inventory control.
• Strong communication and interpersonal skills, with the ability to explain treatment clearly to patients and to collaborate constructively with clinical colleagues.
