Purpose of the Role:
- The role will support the delivery of community-based health services in Gatundu Sub-County by promoting preventive healthcare, conducting health education, facilitating referrals, collecting community health data, and strengthening linkages between households, communities, and health facilities.
Key Responsibilities
Community Health Promotion & Education
- Conduct health education sessions within the Gatundu community on maternal and child health, nutrition, hygiene and sanitation, immunization, family planning, and prevention of communicable and non-communicable diseases.
- Promote positive health-seeking behaviors through household visits, community dialogues, and outreach activities.
Household Visits & Community Engagement
- Carry out routine household visits to assess health needs, provide basic counseling, and follow up on treatment or service defaulters.
- Mobilize community members to participate in Ministry of Health and Kiambu County-led health campaigns and outreaches.
Referral & Linkage to Care
- Identify individuals requiring further medical attention and facilitate timely referrals to designated health facilities within Gatundu and surrounding areas.
- Conduct follow-ups to ensure continuity of care and adherence to treatment plans.
Data Collection & Reporting
- Collect, document, and submit accurate community health data using approved MOH tools and registers.
- Participate in community diagnosis and submit reports to supervisors in line with reporting timelines.
Stakeholder Collaboration
- Work closely with Community Health Committees (CHCs), community leaders, health facility teams, and county health officials to support service delivery.
- Support implementation of government, NGO, and private-sector health programs operating within Gatundu Sub-County.
Compliance & Professional Conduct
- Uphold confidentiality, ethical standards, and professional conduct in accordance with Ministry of Health guidelines and Stratostaff policies.
- Comply with all applicable labor laws, workplace policies, and health sector regulations.
Requirements
- Diploma in Community Health or an equivalent qualification from a recognized institution.
- Minimum of 2 years working experience within a community health setting, preferably in Kiambu County or similar contexts, will be an added advantage.
- Must be a registered Community Health Practitioner and in good standing.
