Monitor and encoding every day of patient dialysis treatment session to Philippine Renal Disease
Registry.
Check and encoding every day dialysis supplies and submit monthly report dialysis machine monitoring
and environmental checklist.
Checking the Guarantee Letter (GL) is addressed to the company and make sure that there are no
missing pages.
Preparing Acknowledgement Letter to be signed by patient, Sales Invoice, Charge Slip, GL and
transmittal summary to be submitted. Follow-up transmittal number with PCSO. Monitoring number of injection usage/dialysis session of the patient with GL.
Assisting the patient with any related issues and Encourage patient to seek help with PCSO every two
(2) months and with DSWD every (3) months.
Preparing Statement of Account and monitor collection. Follow-up collection with Accounting/Treasury
Department weekly and Preparing quotation and certification needed by the patient.
Print forms needed by the nurses like dialysis charge slip, physicians order sheet, etc.
Assist the nurses with documentation, if necessary.
