Overall Purpose of the Job
Reporting to the Senior Provider Relations & Wellness Coordinator, the Wellness Coordinator is responsible for coordinating, initiating and implementing wellness programs to clients. Collaborating with the Care, Claims, Underwriting and Retention team members towards effective execution of the preventive and promotive programs. Conducting Health talks, implementing Wellness schedules & medical camps and development of surveillance and population health tools. Assisting in development and implementation of chronic disease management programs, wellness webpage, employee assistant program, retail and corporate wellness.
Key Responsibilities
Develop guidelines, tools, and indicators for effective disease surveillance at schemes level.
Conduct, in collaboration with other team members, real-time analysis of disease trends based on claims data and advice on appropriate interventions.
Assist the Senior Wellness Coordinator in creating, organizing, and executing programs that promote quality improvement, disease management, population health initiatives, and coordinated care across the healthcare continuum.
Analyze major cost drivers and formulate strategies to guide effective care navigation.
Assess and report on the impact of wellness technical activities and programs, within the context of wellness initiatives and active case management.
Monitoring utilization of schemes and recommend appropriate control measures.
Develop health profiles for individuals and schemes for optimal implementation of care navigation.
Analyze, systematize and disseminate wellness program-specific to support development of Wellness Departmental reports.
Design, monitor and report on the implementation of disease-specific interventions for communicable and non-communicable disease.
Implementation and rollout of Wellness initiatives, ensuring their full implementation and execution within budget; to include Seniors and maternal-child wellness.
Roll out and implement Staff wellness initiatives.
Supervise and coordinate medical camps in collaboration with third party providers.
Maintain accurate and complete records of all client interactions and care plans
Ensure compliance with regulatory requirements related to wellness program design and implementation.
Establish and maintain policies and procedures for protecting the privacy and confidentiality of participant health information.
Participate in audits and compliance reviews related to wellness program governance and effectiveness.
Give reports to management on outcomes of wellness activities on a monthly basis.
Education, Experience & Competencies
Holders of a Bachelors Degree in Clinical Studies (Nursing/Clinical Medicine/Nutrition) or any other related degree.
A minimum of 3 years’ experience in Wellness and/or Health promotion and prevention roles.
Experience in health insurance or similar role is an added advantage
Proficient in the use of Microsoft office suite and packages.
Analytical and problem-solving skills
Strong leadership, interpersonal & communication skills.
Ability to collaborate effectively with internal and external stakeholders.
Agility and flexibility to handle changing priorities.
